Failed to document informed consent including the risks, benefits,and alternatives associated with performing these procedures in an ambulatory surgical facility instead of in a hospital. (Repeat deficiencies)
Failed to document background checks for employees.
Failed to properly store medications. Failed to ensure medications were stored at proper temperature and it was confirmed that all refrigerated medications reviewed were stored outside acceptable temperatures.
Facility failed to keep premises and equipment clean.
