• Expired specimen collection supplies.
• Failed to purchase and maintain the required equipment and supplies needed for emergency situations. There was no oxygen tank available.
  
• Failure to stock crash carts, defibrillator, emergency medications.
• Failure to ensure staff are properly trained.
• Failure to adopt/follow and or/properly review health and safety protocols.
• Failure to accurately document patient records.
• The physician did not perform an ultrasound and document it in the patients file before their second trimester abortion.
  
• Failure to document background screening on the Administrator and Financial Officer. These employees were not included on the clearinghouse roster.
• Failure to provide patients with toll-free number for complaints, neglect, or fraud.
• Administrator failed to ensure that all equipment was maintained and calibrated. Specifically the autoclave, suction machine and defibrillator had expired inspection stickers.
• The facility was no open as advertised to allow an on-site inspection.
