Clinic is Limited to 15 weeks

  • Expired Medications.
  • Failure to adopt, follow, and or periodically review health and safety protocols.
  • Failure to ensure all supplies were within the manufacturer’s directions for safe use. Synthetic sutures were found that had expired in 1996. Expired for 21 years. Multiple expired meds. Caplets of medication were found in cups with no identification labels, no date of expiration or of opening.
  • Unlicensed, unqualified, untrained staff providing patient care. There were no registered nurses on staff.
  • Facility failed to ensure emergency equipment was maintained with adequate supplies capable of providing care. Facility did not have defibrillator pads for use with the defibrillator.
  • Failure to ensure proper patient care post abortion. Patients are supposed to be observed for 60 minutes after sedation and have someone to transport them home. Patients were observed for 22, 17, and 15 minutes (multiple for only 15 minutes) and uterine size and tone were not checked or documented and there was no evidence that patients had transportation other than themselves.
  • Facility failed to accurately document patient records. For several patients reviewed there was no documented evidence that physician performed all required examinations needed before abortion. Five out of 10 patients reviewed were administered undocumented medications.
