• The licensee failed to be responsible for implementing and enforcing written policies governing the facility’s total operation and for ensuring that these policies are administered so as to provide health care in a safe and professionally acceptable environment.
• There was a discrepancy on the narcotics log. Midazolam (Versed) was miscounted on the log.
• The facility failed ensure that policies on decontamination, disinfection, and sterilization, and storage of sterile supplies were implemented. Proper documentation for the sterilizer was not recorded therefore it is impossible to ensure that these instruments that are used from woman to woman were properly sterilized.
• The staff failed to schedule a follow up appointment for all patients who had abortions. They also failed to ensure that every effort was made to reach patients who missed their follow up appointments.
