• The vinyl on the exam table in the sonogram room was ripped which can harbor bacteria and prevent it from being properly cleaned.
• Expired medications and supplies were being used.
• Half of the staff who had direct patient care were not CPR certified.
• Facility failed to provide complete discharge instructions to patients with the phone number of the nearest hospital to the patient’s home in case of an emergency arising from the abortion.
