Clinic is Limited to 15 weeks

  • Facility failed to maintain sterile suturing supplies. Tour revealed several suturing tools and currettes stored in bags in opened containers on a counter in plain view, next to the surgical examination table. At least three currettes had a “build-up of dark colored residue located inside of each loop.” When asked to identify the residue the administrator/physician stated it was not rust which is normal on stainless steel, he then used his fingernail to scrape the residue from the currette where it fell off in a clump onto the counter. When asked if the currettes were sterile, he stated that the currettes were all sterile tools.
  • Facility failed to ensure that preventative maintenance was conducted on all surgical equipment and failure to ensure all surgical instruments were clean and in a good state of repair. Emergency equipment had not been inspected or cleaned, surgical equipment was dirty, dusty, and had dark colored residue on instruments.
  • Failure to adhere to patient care policies and procedures designed to ensure professional and safe care for patients. Patient came in for an abortion, it was performed and patient sent home “in good condition”. Patient called stating she thought she was still pregnant and had a lot of bleeding so she came in, was given a pregnancy test, the test was NEGATIVE but another surgical abortion was performed on patient anyway. Administrator stated that patient consented, but there was no documented consent for the second surgical abortion nor did it contain patient history. Patient had been admitted to the hospital prior to second abortion procedure, when asked if physician received her medical report from the hospital admission, he stated “No, I called and they told me”. There was no documentation that the hospital physician was consulted.
  • Facility failed to accurately document patient records.
  • Failed to document background screenings for Administrator and Financial officer.
