Clinic is Limited to 15 weeks

  • There were no documents in any of the direct patient care employee files that included the job title, education required, licensure requirements, previous experience required, physical demands, performance expectations or any of the components commonly found in a job description
  • There were no performance reviews or competency for any of the direct patient care employees
  • None of the direct patient care personnel were certified in CPR
  • An employee had the job title of “registered nurse” but there was no indication that the employee was a licensed registered nurse.
  • The staff did not report a serious complication and injury of a patient to the appropriate agency. The patient was transported by ambulance to the hospital and required immediate surgery. There was no complication report made.
