• Exam rooms and operating room tables not sanitary.
• Stirrups wrapped in duct tape!!! The clinic administrator explained, “The tape is intended to hold the stirrups in place because some patient’s legs are heavy.”
• No locked cabinet for controlled substances leaving them available to unlicensed personnel
• 75 medical records out in the open overnight putting the privacy and security of the patient’s protected health information at risk of breach.
• Lab testing personnel failed to follow current manufacturer’s instructions for all waived tests performed by the lab as required
