Health Violations

  • Nurse did not follow up with patients who indicated they had a sexually transmitted disease nor did she bring it to the physician’s attention. It also was not reported to the state health department. She also failed to identify the type of STD the patient had nor did she counsel the patients on the transmission of sexually transmitted diseases
  • Staff failed to report the possible abuse of a child. A minor girl, age 13, came to this clinic in January 2016 for an abortion and the facility did not report it. She had a SECOND abortion in April 2017 and no one reported it.
  • Physician did not complete records on patients who came for abortions.
    • no indication of whether pregnancy was viable or nonviable
    • no indication of whether or not the ultrasound was done
    • staff did not document the date of a procedure
    • documentation on patient’s chart was illegible for curettage size and oxygen saturation
    • size of the dilator was illegible on patient’s chart
  • Failed to document orientation on employees
  • Failed to follow it’s own Policy for initial certification of employees
  • Failed to ensure designated staff were certified in CPR
  • Emergency medications found expired
  • Failed to ensure preventative maintenance on clinic equipment
  • Failed to ensure supplies for patient use were not expired
  • Failed to keep procedure rooms clean and free of dust.
