• Inspection found that a patient had an abortion at 18 weeks, complication of cervical laceration and hemorrhaged, was transferred to the ER but her medical record had no documentation of the transfer. Her follow up reference the ER visit APRIL 2014: Another perforated uterus, patient sent to ER.
  • Inspections of Preterm Abortion Clinic have documented a failure to ensure medical records were complete and a failure to ensure training documentation for staff performing ultrasounds.
  • The clinic also has a documented history of botched abortions, including an abortion on March 29, 2014 that resulted in the death of Lakisha Wilson.
  • Failed medication abortions resulted a surgical procedure for patients ( see log below)
  • Ultrasounds being performed by unlicensed and unqualified staff member.
  • There were 19 syringes of Fentanyl that were pre-drawn and not labeled properly with the dosage. The syringes were labeled as containing 1mcg/ml when they actually contained 50mcg/ml. This posed a serious threat to patients. When the inspector questioned the staff member about why the Fentanyl medications are pre-drawn into syringes she stated “It helps facilitate the flow of patients.”
  • The facility was admonished to pay a $10,000 fine for mislabeling these syringes.
  • The physician failed to maintain an accurate and complete record for a patient with a complication on 4/17/19. The patient experienced hemorrhage and emergency medications were administered and the patient was transferred via ambulance to the hospital. The medical record did not reflect the medication administration or the transfer.
  • Another patient had a complication on 4/5/19 and had to be transferred. The medical record was not accurate or complete.
  • Two other patients medical records had not doctor’s signature on the surgical procedure notes form 5/21/19.
  • The facility failed to ensure that a qualified professional trained in infection control directed the infection control program.
  • The facility did not have signs informing women that no one can force or coerce them to have an abortion advising them not to sign the consent form and inform an employee that they are being forced.
