Health Violations

  • Failed to maintain complete and accurate permanent patient records: no consents signed by doctor, no recovery times, meds administered not documented;
  • The doctor performing abortions and administering medications is not registered with the NC Board of Physicians;
  • Doctors not using masks during abortion procedures -Staff not following proper safety control practices;
  • Biohazardous waste stored and stacked in the room with scrubs and personal belongings.
  • The physician responsible for performing the abortion procedures on patients failed to sign a consent form for surgical abortion procedures. This form explains to patients the risks involved in an abortion procedure including death, hemorrhage, infection, failed abortion attempt, organ injury to uterus or cervix, blood clots, and allergic reactions. This form also explains the procedure to the patient.
  • Staff never logged age of gestation and time of procedure for patient charts pulled for inspection.
  • Clinic failed to register physician performing medicine aborions with state board of pharmacy.
  • Untrained staff giving patients injections.
  • Staff failed to audit crash cart to ensure emergency equipment was available for patient use, including the AED (Automatic External Difibrillator).
  • Staff failed to implement infection control practices including not wearing PPE and using dirty and clean medical instruments interchangably.
