The certified nurse anesthetist failed to secure the narcotics used for conscious sedation.
Staff was using single use intravenous fluids on multiple patients.
The staff failed to maintain complete and accurate medical records.
The facility failed to ensure that infection control practices were maintained.
Failed to ensure that instrument loads were sterilized properly. The steam sterilizer documentation failed to note whether the loads passed or failed.
Patient chairs in the recovery area were cloth rendering them unable to be properly sanitized after patient use.
There was a cloth pillow case being used in the recovery room instead of disposable paper.
The staff failed to use proper hand hygiene.
There were no evacuation plans posted in the facility to direct patients and staff to evacuation routes.
Fire drills were not conducted in accordance with the public health code and Connecticut State Fire Prevention Code.
Staff failed to maintain the fire sprinkler system.
Facility failed to maintain the fire alarm system.
Facility ailed to document the correct procedure performed and DID NOT DOCUMENT a complication during the surgery. The patient had to be taken via ambulance to ER for treatment.
