The clinic had no written transfer agreement with the hospital in case of an emergency to ensure continuity of care. The state requires a written transfer agreement in case of complications.
Failed medication abortions resulted in patients returning for surgical abortion procedure
Facility failed to maintain a safe environment. The automatic door was not working properly. Fire extinguishers had not been inspected or serviced.
Walls were dirty.
The facility did not follow oversight procedures for review of the physician’s credentialing and competency. There had been no review for 3 years.
