Clinic is Limited to 15 weeks

• Facility failed to maintain required equipment. There was no policy in place to maintain or inspect autoclave (for sterilization), patient monitoring equipment, or emergency equipment. Interview revealed that cleaning was done in house but the required outside spore growth testing (mold testing) is not done weekly or monthly as instructed by manufacturer.
• There is no documentation or logs proving when or if the autoclave is or has been cleaned.
• No documentation that the emergency equipment has been inspected as required.
• Failure to adopt, follow, and or periodically review health and safety protocols.
