Health Violations

  • HIPAA violation: an employee accessed a patient’s health record without authorization “out of curiosity” Patient requested a copy of their medical record.  When the record was sent to them, the last page belonged to another patient. (HIPAA violation);
  • Patient given wrong log-in info for the web portal and access was granted to another patient’s chart. Facility impeded an investigation by refusing to grant access to medical records to investigators as required by state law (but they breached patient’s confidentiality time and time again);
  • Clinic failed to accurately document in the chart the estimated blood loss on 2 patients who had “copious vaginal bleeding”;
  • Staff failed to verify patient information before handing over prescriptions and urine cups.
