• A staff member violated HIPAA by taking a screenshot of the scheduled patients for the day and texting it to her boyfriend using her personal cell phone to “show him what her schedule looked like for the day”. The picture included patients names, dates, and the reason they were coming into the facility.
• On another occasion, a FAX with patient’s confidential information was sent to the wrong FAX number
• On another occasion a staff member accessed several patient records many different times for no apparent reason except to be nosey and shared the information with others. She admitting to knowing the patient.
• A medical records release form was scanned into the medical record of the wrong patient by a medical assistant. In turn, a medical records clerk copied the wrong record and sent the records to an outside entity.
• These careless errors occur when staff are responsible for too many patients at once and too busy to pay attention to details.
