Clinic is Closed

• Facility failed to ensure that it kept and labeled medications in a proper and safe manner. On a desktop that everyone in the clinic had access to, ten 50ml syringes with clear liquid filled to 20ml line were found. There was clear tape labeled “lido” but no fill date, expiration date, name, initials, or title of who drew up the “lido”.
• Expired medications. Inspection of crash cart found expired medications. Facility policy stated that carts should be inspected weekly and expired meds disposed of. The expired meds went out of date the year before.
• Failure to ensure a safe and sanitary environment. Facility stored medications and patient specimens in the same refrigerator. Medications and urine/blood samples to be used for STD testing were stored in the same fridge.
