• The facility did not have appropriate credentials for the abortion provider’s on file
  • No history and physical on patients before surgical procedures
  • Consents for surgical procedures were signed by a medical assistant and not by a physician. Informed consent must be obtained by the one performing the procedure, not the one assisting.
  • There were two bags of soiled laundry being stored in the recovery area and they had been there for 5 days.
  • Sterile instruments were being stored in the dirty decontamination area. Multiple boxes and supplies were being stored in the dirty decontamination area.
  • Sterilization logs for instruments that are used from woman to woman were not complete. There was no way of tracking the instruments and following up to maintain infection control.
  • The facility administrator could not provide inspectors with fire safety documentation for alarms, sprinklers, lights and extinguishers.
