• Single use patient intravenous fluids were being used for multiple patients
  • The staff failed to maintain complete and accurate medical records (repeat violation)
  • There were expired medications in use that were not labeled or properly stored. The medications were being stored in a refrigerator with patient refreshments (food and drink).
  • The staff failed to follow proper infection control procedures. The administrator noted a “change in management” at that time so “she is not sure” why the procedures had not been followed.
  • Patient chairs had cloth covers preventing them from being properly sanitized after patient use.
  • Facility failed to ensure that medications were properly labeled that were in use.
  • Facility failed to ensure that fire drills were completed per policy. Again, the administrator blamed previous management.
