Single use patient intravenous fluids were being used for multiple patients
The staff failed to maintain complete and accurate medical records (repeat violation)
There were expired medications in use that were not labeled or properly stored. The medications were being stored in a refrigerator with patient refreshments (food and drink).
The staff failed to follow proper infection control procedures. The administrator noted a “change in management” at that time so “she is not sure” why the procedures had not been followed.
Patient chairs had cloth covers preventing them from being properly sanitized after patient use.
Facility failed to ensure that medications were properly labeled that were in use.
Facility failed to ensure that fire drills were completed per policy. Again, the administrator blamed previous management.
