• Facility performed an abortion on a 17 year old minor without a parent or guardian’s signature.
• Patients were not given a copy of their rights and responsibilities as required by law.
• Patients were not given a copy of the complaint procedure and information on how to lodge a complaint
• Physician did not use an aseptic technique (clean the top of the multidose vial before drawing up medication) failing to ensure the prevention of infection
• Failed to have equipment inspected as required by the state
