• A patient was sent home with medication that was labeled with another patients personal health information.
• On another occasion, a medical assistant gave one patient a medication that was intended for and labeled with another patient’s name and information
• On another occasion, birth control pills that were intended for and labeled with another patient’s info were given to the wrong patient
• These careless errors occur when staff are responsible for too many patients at once and too busy to pay attention to details.
