  • There were rusted suction machine cabinets in use posing an infection control issue
  • Bloody single-use plastic tubing attached to the machine’s glass suction canister that was never disposed of after the last abortion procedure 6 days ago
  • Reusable tubing on the top of the suction machine was contaminated with black mold. Staff confirmed that abortions continued to be conducted with that moldy tubing in place.
  • A glass suction container has a dried black substance in the bottom of it which appeared to be mold or dried blood
  • The storage room had a metal cabinet of a second suction machine had numerous rusted areas, old peeling tape, dried adhesive residue on the front surface, (uncleanable surfaces) and a dried brown spill down the side of the machine that was approximately six-inches long.
  • The facility failed to ensure that equipment used for patient care was approved for use in healthcare facilities. Electric heating pads marked for household use only were used in the Recovery Room which had the potential to cause severe burns to patients

This facility’s license expired October 2018. They are no longer licensed to perform abortion services.
