• The clinic did not maintain safe and sanitary conditions.  Instruments were not sterilized properly;
  • Urine samples were not disposed of properly.  A urine sample was found in the pass through that was 4 days old.
  • Expired medications and supplies being used. (repeat violation)
  • An opened bottle of saline was found in use.  Once opened, saline is good for 60 days.  This bottle was opened 2 years ago.
  • The clinic failed to post the State of Ohio’s complaint hotline number, which is required.
  • A medical record review revealed that 2 patients were transferred to ER from the facility but the record did not go with them and there was no documentation that the facility notified the hospital of transfer.
  • The medical records were incomplete and not legible.
  • There was no director of nursing and of the nine nurses on staff, none had operating room experience and were not qualified to fill the role of director of nursing.
  • The clinic failed to provide evidence that one of the physician’s was credentialed to provide clinical services for a period of time.
  • Narcotics were accessible to unauthorized personnel. (The practice manager and security guard)
  • 100% of the medical records reviewed were not accurate or complete.
  • The facility failed to ensure that medication administration was documented correctly.
