- There was no documented evidence that the staff administering IV anesthesia medications were trained, certified or competent
- There was no documented training of staff for emergency transfer of a patient to the hospital from the facility
- The staff was not certified in basic life support (CPR)
- Medical records were not complete and accurate. In 100% of the records reviewed, there was no discharge diagnosis for the patients. –
- Facility failed to conduct fire safety drills as required.(Repeated deficiency)
