• Facility failed to maintain a safe environment. When the inspectors entered the facility, a foul odor was evident immediately. This odor was present throughout the facility
  • Expired supplies were being used, some as old as 4 years
  • A brown paper bag was found in the freezer in the biohazard room. A red substance was leaking from the bag. There 5 ziploc bags inside with fetal remains in them
  • Facility was not following their own policy to record vital signs every 15 minutes on patients in the recovery room
  • No background checks on staff and the facility performs abortions on minors
  • There was no written policy for the discharge of incompetent patients
  • The narcotics were not signed by a pharmacist or physician as required by policy
  • Facility was laundering physicians scrubs, blankets and heating pads on the premises. Soiled linens were not cleaned in a manner to avoid contamination
  • Narcotics were left out in the open in the recovery room when they should be in a locked cabinet
  • There was no application on one of the physicians and no delineation of privileges for anesthesia
  • The facility failed to adhere to professionally acceptable standards to ensure a safe and sanitary environment
