  • Recovery room equipment was not maintained in a sanitary manner. Rust, stains, residue and dust was seen on equipment. Suction tubing that was identified by staff as clean was draped over an open biohazard container.
  • Outdated supplies were in use on the emergency crash cart.
  • In reviewing charts, the surveyors found that two patients were transferred to an inpatient facility with complications but the summary of care given and the suspected complication did not accompany the patient on transfer.
    • One patient transferred for abnormal heart rhythm.
    • Another patient transferred for high blood pressure
  • Staff failed to maintain complete and accurate records. There were no post-procedure counseling notes in some of the charts reviewed.
  • The building did not meet ASC regulations on inspection.
  • This facility failed repeatedly to conduct fire drills and test the sprinkler system as required.
