• Staff received no training on the facility’s policy and procedures. When the administrator was confronted with this information, she stated that she was not aware the staff needed this training.
  • Staff received no orientation did not have experience sufficient to demonstrate competency to perform their jobs.
  • Facility’s policy and procedure manual was not reviewed, revised and approved on an annual basis
  • There were no policies and procedures in place to provide oversight for the center. The facility is expected to ensure that it is in regulatory compliance.
  • The facility was missing policies for pre-operative testing and examination, lab turnaround time, documentation of lab results, review of lab reports, obtaining labs and radiology services to meet the needs of patients
  • There were no job descriptions that included duties and qualifications for 100% of the staff files reviewed.
  • The administrator did not comply with state regulations to ensure that all staff were free from communicable diseases by administering a TB test and hepatitis test/vaccine. This put all patients and staff at risk and shows a disregard for their welfare.
  • The ultrasound and sterilizing machine had no preventative maintenance and inspection to ensure it was working properly
  • One of the physician’s working in the practice had no resume on file showing his education, board certification, post graduate training, any hospital where he has appointment or employed in the last ten years, and any disciplinary action. This is a requirement of the state.
  • Both of the physicians in the practice had incomplete files with missing credentials.
  • None of the staff were certified in basic life support (CPR)
  • Facility has no AED to be used in case of emergencies
  • There is no equipment to monitor the patient’s oxygen level.
  • Expired medications and supplies were in use
  • The RN did not discard expired supplies, did not implement infection control policies, and did not ensure that measures to prevent infection were in practice at the facility.
  • Instruments being used from woman to woman were not being sterilized properly
  • Examination table had cracks in the upholstered cover and were covered with tape.
  • Drawers in the exam table were dirty with black debris
