Health Violations

  • The facility did not report infectious and communicable diseases to the Arkansas Department of Health.
  • The facility failed to identify infectious diseases in patients and workers in order to prevent the spread of such diseases.
  • The facility failed to provide linen care had the potential to allow cross contamination between patients. Three of three blankets inspected had not been washed in between patients.
  • No evidence that the Medical Director or a licensed nurse had performed a monthly compliance assessment for drug storage, disposal of drugs, proof of receipt and administration of controlled substances and their storage, drug labeling, monitoring of floor stock and maintenance of emergency carts or kits.
  • The narcotics count was not accurate.
  • Inspectors found expired medications available for patient use in the nursing station.
  • The facility failed to have a policy in place which included a current list of stock medications with minimum and maximum quantities to be maintained in the facility. Without this policy in place the facility would not be able to assure that they have enough medications on hand to address the needs of their patients.
  • The facility failed to have a policy in place for reporting suspected drug loss, misuse, or diversion.
  • Cloth chairs were in use in the recovery room and one had a rip in the seat. This material is absorbent and unable to be cleaned and sanitized to prevent to spreading of infection or communicable diseases.
  • The facility was not clean or maintained. The inspectors found an accumulation of dust on equipment, brown and stained ceiling tiles, missing ceiling tiles, and trash on the floor.
  • Staff did not properly sanitize and sterilize equipment and instruments that are used from woman to woman.
