Health Violations

  • There was no documentation provided by the facility to show that the physician was board certified in Obstetrics and Gynecology;
  • Facility failed to ensure that all patients, prior to their procedure, had blood drawn for ABO blood group for 12 of 12 records;
  • Facility failed to employ a registered nurse or have one in attendance during the hours patients were present;
  • Background checks were not completed for the employees. Further interview confirmed that the facility admitted and treated individuals under the age of 18;
  • Facility did not conduct Quality Assurance meetings or track patient data to identify opportunities for improvement in the care provided. No performance indicators selected, no data collected for the pediatric population served and no active surveillance for post-procedural infections.
  • Administrator failed to develop/implement policies and procedures for safety practices.
  • Facility failed to provide emergency training for patient transfers to a hospital.
  • Failure to properly label and store medications. Repeated deficiency
  • Failure to accurately document patient records. Patient records did not include discharge diagnosis. Repeated deficiency.
  • Failure to train, comply with, and ensure infection control standards.
  • Failure to ensure sterilization of surgical instruments. Repeated deficiency
  • Failure to ensure staff are properly trained.
  • Failure to adopt, follow, and or periodically review health and safety protocols.
  • Unlicensed, unqualified, untrained staff providing patient care. Facility failed to credential medical staff.
  • Expired medications and surgical supplies.

**A follow up inspection found that the facility failed to comply with any plan of correction and continued in their deficiencies**
