Clinic is Limited to 15 weeks

• Failed to document background screenings on the Administrator and the Financial Officer. (repeated deficiency)
  
• Failure to properly train employees.
• Unlicensed/unqualified/untrained staff providing patient care.
• Failure to purchase and maintain proper required equipment. Facility had no hot water, so it was impossible to maintain sanitary conditions.
• Failure to accurately document patient records. (Repeated deficiency)
  
• Patient records had no recovery period documented. When the inspector asked the administrator why there was no documentation of the recovery period for these patients, she said the documentation was probably locked up and she did not have the key.
