Health Violations

• Expired medications and supplies in the emergency bag and IV supply box (some over 5 years ago). RN states that the box is checked every month.
• No documentation of patient’s recovery time (minimum of one hour)
• Clean and dirty instruments stored on the same shelf
• 7 of the 10 recliners in the recovery room had tears in the seat
• Staff never documented patient pain levels from surgical abortions in 100% of patient charts pulled for review.
• Clinic used outdated templates for patient charting that did not have required charting information on them such as bleeding and pain management.
