Clinic is Limited to 15 weeks

• Failure to train, comply with, and insure infection control standards.
• Failure to adopt, follow, and or periodically review health and safety protocols. (Repeated deficiencies)
• Failure to ensure staff are properly trained.
• Expired medications and supplies. Meds on emergency carts were out of date (some expired by three years!)
• Failure to properly label and store medications. Multiple baskets of pre-filled syringes not properly labeled with post-it notes being used to label. Multiple pill bottles containing 4 or 10 pills with no labels next to empty prescription bottles for various drugs.
