Clinic is Limited to 15 weeks

• Expired medications and supplies. Crash cart was stocked with out of date medications. (Repeated deficiencies)
• Failure to adopt, follow, and or periodically review health and safety protocols.
• Failure to ensure staff are properly trained.
• Failure to maintain required equipment. Mechanical suction machine showed no evidence of ever being inspected or routinely maintained. Failed to provide a plan of correction.
• Failure to document background check on Administrator and Financial officer.
