Clinic is Limited to 15 weeks

• Failure to provide toll-free number to patients for reporting complaints, neglect, or fraud.
• Failure to allow patients to view ultrasound images before abortion.
• Failure to explain ultrasound, provide information on baby’s stages of development, alternatives to abortion, services available such as medical assistance, prenatal care, birth care, and child care provided by AHCA (Agency for Health Care Administration). Patients had requested on forms to view ultrasounds before having an abortion and clinic didn’t offer it.
• Failed to document informed consent.
• Failed to ensure sonographer was properly certified and trained
