In 100% of the charts reviewed, the clinic failed to report the abortions performed as required by the health department. This report includes age, gestation of the unborn child, and any complications.
Failure to report services provided to patients.
Failure to accurately document patient records.
Attending physician failed to perform required blood count and urine tests on patients prior to performing abortions.
The facility had no approved patient safety plan and did not conduct patient safety meetings for oversight.
