• In 100% of the charts reviewed, the clinic failed to report the abortions performed as required by the health department. This report includes age, gestation of the unborn child, and any complications.
  
• Failure to report services provided to patients.
• Failure to accurately document patient records.
• Attending physician failed to perform required blood count and urine tests on patients prior to performing abortions.
• The facility had no approved patient safety plan and did not conduct patient safety meetings for oversight.
