Health Violations

  • The doctor failed to report a botched abortion to the public health and safety department, thus not allowing for any oversight and accountability
  • The staff failed to monitor and document vital signs and degree of bleeding;
  • The facility does not have a counseling training policy and procedure in place. Failure to have a counseling policy and procedure may put patients health and safety at risk as they may not be provided with accurate procedure information by qualified staff
  • The physician did not have a policy to define what is a stable discharge which puts all patients at risk if patients are discharged before fully recovering from the abortion procedure and sedation
  • Staff failed to monitor and document a patient’s vital signs and degree of bleeding. There was NO DOCUMENTATION of CARE provided to the patient while in the recovery room for 47 minutes
  • The facility did not have a policy for infection control for sterilization and sanitation of instruments used from woman to woman
  • Facility failed to report that a woman had a perforated uterus during her abortion procedure and had to be transferred to a hospital. This complication should have been reported to the state department
  • Half of the nurses on staff did not have Advanced Cardiac Life Support certification that is required, A patients quality of life may be altered if they have a pre or post emergency procedure and the nurses are not currently trained in ACLS procedures
  • 2 medical assistants and 1 RN failed to have CPR certification. When the nurse and medical assistants do not have current training in CPR it poses a risk to a patient’s quality of life when the staff may not able to provide immediate emergency assistance if needed.
  • The facility failed to file an application for relicensure within 60 days of the expiration date of the current license. The facility paid a fine of $250 (repeat violation).
