• Staff failed to identify fetal tissue that was stored in the freezer. The clinic had no policy for identifying an labeling fetal tissue. The specimens were wrapped in a chux and placed in a bag.
• Expired dilators and laminaria were in use, as old as 12 years
• There was no evidence of preventive maintenance on the ultrasound machine, defibrillator/cardiac monitor, pr suction machines.
• The autoclave maintenance (weekly, monthly and Maxi) which is required to ensure it is working properly was not documented. The log book was available but there was only documentation for January. This inspector was present in August.
• This facility is owned by Dr. Leroy Carhart, who performs late term abortions.
