Health Violations

  • Failed to notify patients that the doctor did not have admitting privileges at a local hospital as required by law.  On their surgery screening sheet, there is an information line that states “our physicians have privileges at hospitals within 30 minutes of our facility that offer OB care”.  This was checked and forms signed;
  • MD #2 performed the procedures and has no such privileges.;
  • No RN present on surgical days for patient care;
  • A record reviewed revealed that two had surgical abortions on 4/17/15 and  5/1/15 with no complications documented.  Surgery record was signed by non-licensed staff. Both patients ended up in the ER the next day and it was revealed that each woman had an INCOMPLETE ABORTION.
