CLOSED

• Clinic failed to have pain and comfort medications available for patients having surgical abortion procedure.  The doctor left the key to the cabinet in Selma. All 17 abortions performed that day were done without sedation medication;
• Clinic staff failed to report reasonable suspicion of abuse as required by the state when they performed an abortion on a 14 year old minor at 16.5 weeks gestation. There is no policy in place reflecting the state’s mandatory reporting for abuse or neglect of a minor;
• Medical Director and Physician performing abortions unaware of mandatory reporting requirements for abuse or neglect of a minor;
• The clinic had no medical waste picked up for a total of 8 months because they owed Stericycle a lot of money and they would not pick up until paid. The clinic admitted to placing the medical waste in black trash bags and disposing of it in the dumpster.
