  • Facility failed to implement a complaint process and designate staff for complaint resolution.
  • There was no verification of credentials/certification for 5 of 7 employees who were supposed to be licensed
  • This facility failed to comply with state regulation regarding handling and distribution of controlled substances
  • All seven employee records had no TB screening or Hep B screening/vaccination. This shows a lack of regard for the health and wellness of the employees as well as the patients entering the facility
  • No signed contract from the physician performing abortions
  • No documentation of the doctors education or training which qualified him to perform abortions.
  • Physician did not remain on the premises until all patients were stable and discharged
  • No informed consent obtained for a minor having an abortion
  • Staff failed to give patients a complaint form in case they have a complaint or a 24 hour emergency number if there is a complication. This is in violation of state regulation
  • Failed to implement an infection control policy
  • Chairs and recliners in the recovery room were not cleaned and had rips in the vinyl
  • Staff violated the policy by not documenting patients condition and verifying that instructions were given for signs of infection in 70% of charts reviewed
  • No proper documentation of handling and inventory of controlled substances. Staff excuse was “there is not enough time”
