Clinic is Limited to 15 weeks

  • Failure to ensure staff are properly trained.
  • Failure to adopt, follow, and or periodically review health and safety protocols.
  • Failure to follow protocols for safe medication practices.
    • Facility did not date pre-poured medications.
    • Syringes full of meds were stored in a jar with no dates and some with expired meds in them.
    • Med cabinets were found to be left unlocked in the recovery room.
  • Expired and damaged medications and containers.
  • Failed to provide patients with toll-free number to call for complaints, neglect, for fraud.
  • Failure to ensure a safe and sanitary environment. Opened and uncovered biohazard containers were noted.
  • Failure to follow up with patient 24 hours after a 2nd trimester (19-20 week) abortion to assure recovery and or no complications as required by law.
  • Failure to accurately document patient records. Failure to note patient’s condition after procedure with physician’s signature on incomplete records.
  • Failure to document background screenings on employees, specifically the clinic director and CFO.
