• Failed to ensure a safe and sanitary environment. A basket filled with (27) 10 mL syringes was found with no labels. No information as to what the syringes contained, no dates of preparation or expiration, and no label of who prepared the syringes. Investigation found facility had no policies or procedures relating to medication preparation, and storage.
