• Failure to ensure emergency equipment was available for immediate use for inhalation and suctioning. There were no supplies stocked in the clinic for patient use if the breathing machine was needed.
• Failure to purchase and maintain proper medical equipment.
• Failure to ensure staff are properly trained.
• Unlicensed/unqualified/untrained staff providing patient care.
• Expired medications and supplies. Surveyor noted 443 expired supplies and medications in three procedure rooms.
• Failure to ensure a safe and sanitary environment.
• Failure to assess patient post abortion to ensure recovery after procedure/sedation.
• Failure to accurately document patient records including what medications were given and when, if at all, as well as patient history, and treatments administered.
