  • Failure to document informed consent.
  • Failure to comply with state follow up safety procedures after an abortion procedure.The physician stated that follow up phone calls were not made to patients “in an attempt to maintain their confidentiality” .
  • Facility failed to follow up at all to assess recovery, as required.
  • Failure to properly maintain medical equipment.
  • Failure to ensure a safe and sanitary environment.
    • Tools and equipment were found “caked with rust” brushes used to clean surgical tools were “soiled with brown and white matter” linens were stained “brown material”.
    • Dishes and jars containing products of conception were soiled and sitting with the soiled brushes.
  • Failure to maintain required medical equipment.
  • Failed to report an incident that resulted in serious injury to a patient. A patient’s bowel and uterus was perforated and required hospital intervention. Patient needed bowel resection and hysterectomy. Further inspection discovered a log noting ‘failed abortion procedures and complications” that had six unreported uterine perforations noted.
