Health Violations

• Ultrasounds performed by personnel who have no certification or documented training in the use of the ultrasound equipment
• Expired supplies and materials in use (sterile suturing equipment, urine test strips, hydrogen peroxide, rubbing alcohol) REPEAT VIOLATION
• Medical records of abortion patients stored in an unsecure area, in a room used for counseling and exams.
• An uncovered disposable oxygen mask was hanging from an oxygen canister and touching a trash can containing a biohazard bag. There was not a replacement and the administrator confirmed that this mask was “cleaned and reused between patients”.
• Medications were not kept secure and locked.
• Suction machine used for evacuation has right side unsecured to cabinet
• Cabinet found with peeled paint and rust throughout
• Medication found unlabeled, not stored properly, expired.
