Health Violations

• Clinical staff failed to administer Rhogam to an Rh negative patient who had an abortion;
• Staff failed to follow policy for proper hand washing;
• Clinical staff failed to maintain an accurate and complete record; no documentation of IV and foley access time, size, removal, etc;
• Ultrasound machine documenting the incorrect time on ultrasounds;
• Expired medications in use;
• Patient not offered the opportunity to view her ultrasound;
• Medical equipment not cleaned between patients.
