Health Violations

  • Failure to ensure that each patient was treated with consideration, respect, and full recognition of dignity and individuality. This inspection was the result of a complaint investigation
  • The reception was short and rude with patients. The facility did not meet the standards of safety and cleanliness.  There was trash in the waiting room, stains on the carpet, dust on the equipment
  • A drawer containing a physician prescription pad was left unlocked and open
  • Operating room had not been cleaned or sanitized in four days
  • The patient holding area had not been vacuumed in four days
  • There was no adequate quality improvement and  assessment program in place Failure to meet the standards for safety and sanitation
  • Surfaces in the operating room were noted to be dusty with white residue. The dusty white residue was noted on the drawers of the respiratory support cart, on the light above the operating room table and on the tops of the monitoring equipment
  • Three large red biohazard barrels approximately 32 gallons in size were stored in the recovery area. A staff member confirmed that the barrels were filled quickly on busy surgical days and maintained in the area for “convenience”.
