Clinic is Limited to 15 weeks

• Failure to ensure background screenings had been run on all employees.
• Unlicensed/unqualified/untrained staff providing patient care.
• Clinic failed to employ any physician’s assistant, registered nurse, licensed practical nurse, nurse practitioner, or any other nurses. The only licensed or certified staff member was the physician who performed the abortions. The rest of the staff providing patient care held no formal training, certifications, or licenses and were only trained in house.
• Failure to adopt/follow/and or periodically review health and safety protocols.
• Failed to maintain current CPR certification for staff working in the recovery room
