- Facility failed to ensure a sanitary environment. Inspectors found standing water in a bucket, red stains on the wall, and loose debris on the floor in the surgical suites. The last surgical day was 2 days prior.
- Patients did not receive post-operative counseling
- Staff failed to maintain an accurate and complete medical record
- The fire sprinkler system was not functional and the facility did not have fire resistant walls as required by safety code.
- Fire alarm system inspection was not properly documented.
- Facility failed to properly store portable oxygen containers. The liquid oxygen containers were less than 20 feet from combustible items.
- Sink fixtures were not hands free which posed an infection control risk to all staff and patients.
- Electrical outlets were not on a GFI circuit in the sterile processing room or the POC (products of conception) lab.
- The abortion facility was also performing vasectomy procedures but was not licensed to do so. This is a repeat violation (2011, 2015)
- Facility failed to ensure successful sterilization of instruments that are used from woman to woman, failed to maintain sterilizer logs, failed to ensure surveillance of the sterilizer process
- Facility’s infection control officer failed to ensure the surveillance of sterilization was conducted according to policy
- The facility failed to ensure that narcotics were secure at all times. The inspector observed the anesthesiologist place a tray containing controlled substances on a desk at the entrance/exit to the recovery room. The medications remained unattended for 45 minutes until a nurse locked them up.
- The facility failed to ensure patients were evaluated by a nurse before discharge to assess the patient’s overall condition and document in the medical record that the patient is ready for discharge
