Health Violations
- For four sampled patients there was no date recorded for the last menstrual period, however, the ultrasound images reflect gestational age in excess of 12 weeks
- For two sampled patients the last menstrual period recorded was in excess of 14 weeks, and the ultrasound reflected a gestational age in excess of 12 weeks.
- The clinic was providing services beyond 14 weeks which is illegal. The center was only licensed for up to 14 weeks.
